Not registered with a GP yet?
Hawthorn Medical Centre is able to register patients who live in the Levenshulme Ward Boundary. (Click here for a map). If you live outside of this boundary we will not be able to accept you as a patient. For help choosing a GP surgery please visit the NHS Choices website
How To register
In order to register with Hawthorn Medical Centre you will be required to complete two forms and attend a new patient health check with a member of our clinical team. The two forms can either be obtained from reception or downloaded from this page. If you download the forms you will need to either telephone the surgery or call in to make the new patient check appointment.
During this new patient check we will take your height / weight and then take a brief medical history from you. It's nothing to worry about, we simply want to get to know you and your health a little better. the check usually takes around 20 minutes to complete and once its done you will be registered with the surgery and be free to make appointments with our Doctors or clinical staff.
We register all new patients with the preactice and not an individual GP which gives patients the choice over which GP they see. It may mean that on some occasions you may have to wait a little longer if you wish to see a particular GP. If you would like to see a particular GP please state this at the time of booking.
What to bring with you to your appointment
When you come to your new patient check we ask that you bring a few things with you
- The two registration forms fully complete and signed if you didn't leave them with us
- A urine sample (you can either take away a pot and bring it back with you otherwise you will be asked to provide one on the day)
- Any medication you are on
- Two forms of identification - One form of personal indentification and one proof of address
For children under 16 we will ask to see the documents of one of the parents.
We may not be able to register you if you cannot provide us with some form of identification and proof of address. If you cannot prvide it on the day we will give you 10 working days to provide it otherwise you may be removed from the practice.
If you were born outside of the UK we also need to know the date you entered the UK.

- passport or identity card
- Driving license*
- NHS Medical Card
- Birth certificate
- Marriage certificate
- Student I.D card
Proof of Address
- Bank statement
- Council tax statement
- Utility bill (dated within last 3 months)
- Rent book / tenancy agreement
- Wage slip (if it includes home address)
- Official letter (Home Office / DWP)
*Driving license and Medical Card cannot be used as both proof of address and as identification
